Introduction:
Cholecystocutaneous fistula is a rare complication of longstanding gallstone disease, with spontaneous cutaneous extrusion of gallstones representing an exceptionally rare and striking clinical manifestation.
Case Presentation:
An 80-year-old female with a long history of cholelithiasis and recurrent cholecystitis presented with abdominal pain and was initially managed conservatively due to poor general condition. Imaging revealed gallstones and pericholecystic inflammation extending toward the anterior abdominal wall. Over time, she developed a cholecystocutaneous fistula, followed by spontaneous extrusion of multiple gallstones through the skin. Seven large gallstones, the largest measuring approximately 4 cm, were removed under local anesthesia. Further imaging confirmed residual gallstones and a fistulous tract between the gallbladder fundus and the abdominal wall, with no evidence of malignancy. The patient was managed with antibiotics, ursodeoxycholic acid, and local wound care using a stoma bag. The fistula output progressively decreased and closed spontaneously within one month. Despite recommendation, the patient declined surgical intervention. During follow-up, only mild intermittent symptoms were observed without progression to a complicated condition.
Discussion:
Cholecystocutaneous fistula is an uncommon entity in modern clinical practice due to advances in early diagnosis and treatment of gallbladder disease. While surgical management remains the definitive treatment, conservative management may be appropriate in selected high-risk patients. Spontaneous closure of the fistula, although rare, can occur when adequate drainage and infection control are achieved.
Conclusion:
This case highlights that spontaneous extrusion of gallstones is an exceptionally rare presentation of cholecystocutaneous fistula. In carefully selected patients with poor surgical candidacy, conservative management may result in successful fistula closure and favorable outcomes.

Giriş:
Kolesistokutanöz fistül, uzun süreli safra taşı hastalığının nadir bir komplikasyonu olup, safra taşlarının cilt yoluyla spontan olarak dışarı atılması bu durumun son derece nadir ve dikkat çekici bir klinik prezentasyonudur.
Olgu Sunumu:
Yirmi yıllık kolelitiazis öyküsü ve tekrarlayan kolesistit atakları bulunan 80 yaşındaki kadın hasta karın ağrısı ile başvurdu ve genel durumunun kötü olması nedeniyle başlangıçta konservatif olarak tedavi edildi. Görüntüleme yöntemlerinde safra kesesinde taşlar ve safra kesesi fundusundan ön abdominal duvara uzanan perikolesistik inflamasyon saptandı. İzlem sürecinde hastada kolesistokutanöz fistül gelişti ve bunu takiben çok sayıda safra taşı cilt yoluyla spontan olarak dışarı atıldı. En büyüğü yaklaşık 4 cm olan toplam yedi taş lokal anestezi altında çıkarıldı. İleri görüntülemelerde safra kesesinde rezidü taşlar ve safra kesesi fundusu ile ön abdominal duvar arasında fistül traktı saptanırken malignite lehine bulgu izlenmedi. Hasta antibiyotik, ursodeoksikolik asit tedavisi ve stoma torbası ile lokal bakım ile takip edildi. Fistül drenajı giderek azaldı ve bir ay içerisinde tamamen kapandı. Cerrahi tedavi önerilmesine rağmen hasta kabul etmedi. Takiplerinde yalnızca hafif ve aralıklı semptomlar gözlendi, komplike bir tablo gelişmedi.
Tartışma:
Kolesistokutanöz fistül, günümüzde safra kesesi hastalıklarının erken tanı ve tedavisi sayesinde nadir görülen bir durumdur. Cerrahi tedavi standart yaklaşım olmakla birlikte, yüksek riskli hastalarda konservatif tedavi uygun bir alternatif olabilir. Yeterli drenaj ve enfeksiyon kontrolü sağlandığında fistülün spontan kapanması mümkündür.
Sonuç:
Bu olgu, safra taşlarının spontan cilt yoluyla dışarı atılmasının kolesistokutanöz fistülün son derece nadir bir prezentasyonu olduğunu göstermektedir. Cerrahi açıdan yüksek riskli hastalarda dikkatli izlem ile konservatif tedavi başarılı sonuçlar sağlayabilir.
