Using of Pedicled Rectus Abdominis Musculocutaneous Flaps in the Thigh and Lumber Defects

A series of previously described but rarely used variation of the pedicled extended or vertical rectus abdominis musculocutaneous flap (Extended RAM, VRAM) was reviewed. Skin paddle dimensions ranged from 8 to 28 cm in width and from 10 to 35 cm in length in 5 consecutive patients. Four flaps were placed through deep to the inguinal ligament to repair the thigh as far as the proximal to the knee region; the remaining one flap was passed transabdominally to cover the defect of the lumbar region. No flaps necroses were seen, and one case wound healing problems required minimal operative intervention.
Successful transfer of the VRAM and extended RAM with low rate of complication for the thigh and lumbar region defects was demonstrated to be safe and reasonable option of flap reconstruction. The flaps had the advantages of being robust and a well vascularized flap, easy and fast to harvest, and not requiring microsurgery experience.

AMA�: Daha �nce tan�mlanm��, fakat az kullan�lan �ekstended� veya vertical rektus abdominis muskulokutan (�Ekstended� RAM, VRAM) fleplerinin �ok nadir endikasyonlardaki kullan�mlar� incelendi.
GERE� VE Y�NTEM: Be� hastada, 28x8 ile 35x10 cm ebatlar�nda cilt adas� i�eren flepler kullan�ld�. D�rt flep uyluk ve diz proksimal b�lgedeki defekt onar�mlar� i�in inguinal ligamentin alt�ndan, bir flep lomber g�lgedeki defekt onar�m� i�in transabdominal olarak defekt alanlara ta��nd�.
BULGULAR: Hi�bir olguda flep kayb� g�r�lmedi. Bir olguda yara yeri enfeksiyonu geli�ti ve cerrahi ile tedavi edildi. Uyluk ve lomber b�lge defektleri onar�mlar�nda d��k komplikasyon oranlar�yla �ekstended� RAM ve VRAM fleple ba�ar�l� transfer ger�ekle�tirildi ve �ok g�venli ve uygun bir se�enek oldu�u sunuldu.
SONU�: Avantajlar� bol miktarda ve iyi kanlanan doku ta��mas�n� sa�lar, flep kolay ve h�zl� haz�rlan�r ve mikrocerrahi deneyimi gerektirmez.
